What happens after you refer

1. Submit the referral form below, or email us directly
2. We acknowledge receipt, typically within 2 business hours
3. We contact the participant directly to arrange a needs assessment
4. We confirm the support plan and start date with you and the participant
5. Ongoing updates are provided to you, with the participant's consent
