We want to hear from you

Whether it's positive feedback or a concern, we want to know. Raising an issue never affects the support you receive from us.

How to give feedback or make a complaint

  1. Use the form on this page
  2. Speak with your support worker or coordinator directly, if you're comfortable doing so
  3. Call us on 0433 604 507
  4. Email us at thehealthwellbeinghub@gmail.com
  5. Write to us at 73 Jacaranda Avenue, Logan QLD 4114

You're welcome to raise a complaint in Arabic, Somali, Dari or Amharic — let us know your preferred language and we'll connect you with a team member who speaks it. You can also ask a family member, advocate or support coordinator to raise it on your behalf.

What happens after you raise a concern

  • We'll acknowledge your complaint and confirm we've received it
  • We'll look into what happened and, where appropriate, discuss it with the people involved
  • We'll get back to you with an outcome or next steps
  • We'll keep a record of the complaint and how it was resolved, in line with the NDIS Practice Standards
